According to Professor Sri, the conventional reliance on one or two methods is inadequate against a disease that is becoming increasingly pervasive. She advocates for an integrated strategy that harmonizes three critical pillars: rigorous vector control, enhanced clinical diagnosis at the primary healthcare level, and the widespread adoption of medical innovations, specifically vaccination.
"Facing the threat of dengue, which is so severe and widespread, we can no longer rely solely on one or two conventional prevention efforts like 3M Plus. We need something comprehensive," Professor Sri stated. She further elaborated that protecting the most vulnerable populations, particularly children, requires a "shield" built from both environmental management and biological immunity. This comprehensive approach is designed to close the gaps left by traditional mosquito eradication programs, which often struggle with consistency and public compliance.
The Staggering Economic Toll of Dengue
The call for a revamped strategy is backed by sobering data regarding the socio-economic impact of the disease. A comprehensive study conducted by Universitas Gadjah Mada (UGM) projected that by 2024, the total economic burden of dengue in Indonesia would reach approximately USD 550.9 million, or nearly Rp9 trillion. The UGM study further estimated that the disease would result in more than 2 million hospitalizations annually across the country. The modeling suggests that a consistent implementation of dengue vaccination over a 20-year period could drastically alter the disease’s trajectory in Indonesia.
According to the projections, vaccination could prevent between 1.1 million and 1.3 million Disability-Adjusted Life Years (DALYs). The DALY is a critical metric used by the World Health Organization (WHO) to quantify the burden of disease, representing the number of years lost due to ill-health, disability, or early death. By reducing the DALY count, vaccination not only saves lives but ensures that more citizens remain healthy and productive contributors to society.
From a fiscal perspective, the modeling indicates that a 20-year vaccination program could yield cost savings ranging from USD 329 million to USD 731 million (approximately Rp5.2 trillion to Rp11.5 trillion). These savings stem from the reduction in symptomatic cases, fewer emergency room visits, and a decrease in the occupancy of hospital beds, which allows healthcare resources to be allocated to other pressing medical needs.

Dr. Rismala Dewi, Chairperson of the Indonesian Pediatric Society (IDAI) for the Jakarta Chapter, pointed out that children often suffer the most severe complications from dengue. The IDAI has been proactive in updating clinical guidelines to ensure that doctors across the country can identify early warning signs, such as persistent vomiting, mucosal bleeding, or extreme lethargy, which indicate that a case is becoming critical. However, Dr. Rismala stresses that prevention through vaccination and environmental control is far more effective than treatment after a child has already become ill.

Andreas Gutknecht, President Director of PT Takeda Innovative Medicines, emphasized that every individual in Indonesia is at risk of infection, regardless of their socio-economic status or geographic location. "We remain committed to working with various stakeholders to support comprehensive dengue prevention efforts through continuous education and expanding access to innovative prevention," Gutknecht said. The goal is to empower families to take proactive steps before an outbreak occurs, rather than reacting to one.

Climate change has led to warmer average temperatures and unpredictable rainfall patterns in Indonesia, creating ideal breeding conditions for the Aedes aegypti mosquito throughout the year. Furthermore, rapid urbanization has resulted in high-density living environments where water storage is common, providing ample opportunities for mosquitoes to thrive in close proximity to human hosts.

Countries like Brazil and Thailand have also begun integrating vaccination with high-tech mosquito control methods, such as the release of Wolbachia-infected mosquitoes, which are unable to transmit the virus.
